Glioblastoma multiforme with granular cell differentiation, intraoperative smear x 400.
Granular differentiation and malignant astrocytic tumors may resemble macrophages on intraoperative smear preparations. Macrophages generally demonstrate more foamy cytoplasm, while granular cells demonstrate coarse eosinophilic granularity such as is seen here.
