This is a cross section of a carotid artery, showing severe atherosclerotic changes (recall that the term atheroma is derived from the Greek word for "meal", in the sense of porridge). As opposed to the coronary arterial circulation where occlusion often occurs secondarily to superimposed thrombosis, critical carotid artery stenosis is most often secondary to hemorrhage within the atheromatous plaque with consequent luminal compromise.
