CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy) is a hereditable systemic arteriopathy involving small blood vessels throughout the body. Clinical manifestations are secondary to multiple small white matter infarcts which produce a subcortical dementia resembling Binswanger's disease in relatively young, normotensive, patients. Ultrastructurally, electron dense deposits are seen within the vascular smooth muscle. In severely involved vessels, granular PAS positive material can be seen within the walls of small penetrating white matter blood vessels.
